There was a bag of trash that was accessible to children outside on the playground as well as multiple protruding nails from the fence and a broken fence picket. Also outside was a rake that was accessible to children and there were multiple tall weeds/grass. Inside the operation, multiple table tops had not been cleaned as they had food or bubble residue on them and there were multiple spots throughout the operation that the floor needed to be swept or mopped due to dirt/debris and food.
