During the inspection, a nail was observed protruding from the fence on the playground, two fence pickets on the playground were broken on the bottom causing a shear hazard, there is a plastic flower pot that is broken that has sharp edges, there is a bag of trash on the ground and there is dirt and leaves on the floor in the hallway.

There were three instances when the caregiver left the classroom with some or all of the children remaining in the classroom alone. This prevented the caregiver from having the needed physical proximity to the children if a situation were to arise and they needed to intervene.

There was a bag of trash that was accessible to children outside on the playground as well as multiple protruding nails from the fence and a broken fence picket. Also outside was a rake that was accessible to children and there were multiple tall weeds/grass. Inside the operation, multiple table tops had not been cleaned as they had food or bubble residue on them and there were multiple spots throughout the operation that the floor needed to be swept or mopped due to dirt/debris and food.
