Supervision is the function of observing, overseeing, and guiding a child or group of children, including an awareness of, and responsibility for, the ongoing activity of each child, and being near enough to intervene when needed. The program is required to maintain supervision at all times.

Licensing and CW observed personnel yelling. Licensing asked personnel what was going on. Licensing was informed that a child was left outside on Sept 16th.

Licensing observed infant teacher walking down the stairs and into the classroom and the teacher in room exiting. Licensing walked in behind the infant room teacher and observed 2 infants (6 & 9 months) asleep in swings with blankets covering the children. Personnel stated that one infant asleep is swing for maybe 10 minutes. Other infant asleep for maybe 5 minutes. Infant teacher was gone for about 5 minutes to use the restroom.

Non-compliance for -One staff hired on 5/29/2025 did not have ELCCT completed documented on 10/9/25 monitoring visit with POC of 10/13/25. Licensing observed personnel's order history on personnel's cecpd account and ELCCT was purchased until 10/14/25. Licensing observed personnel has not completed ELCCT training.

Licensing observed diaper cream in children's cubbies in 3 different classrooms without a medication permission form filled out or on file for the children.

Licensing observed 2 walkers in the gym and one inside the program. Licensing asked personnel what the walkers were used for in the gym. Licensing was informed infants use them when in the gym.

Licensing observed 3 teachers that have not obtained or maintained CPR/FA within 3 months of employment. Licensing observed 1 personnel that CPR/FA expired 4/21/24 and has not been obtained or maintained.

Licensing observed 1 personnel with an expired PDL. PDL expired on 7/15/24 and was submitted for renewal on 7/26/24.
