Fire inspections. Inspections are conducted at least every two years by the local or state fire governmental authority having jurisdiction. Documentation is maintained per OAC 340:110-3-281.2(c).

Fire inspection has not been conducted within the last 2 years. The fire inspection was last conducted on 4/5/24.

Ratios. Ratios are the number of teaching personnel required to be physically present with a child or group of children, per Oklahoma Administrative Code (OAC) 340 Appendix GG - Ratios and Group Sizes. Only teaching personnel count in ratios, unless requirements specifically state otherwise. The program is required to maintain ratios at all times.

Licensing and Child Welfare observed school age bus to arrive at the center at 3:00pm and the school age children went outside to combine with the 3 year old and older classroom. Licensing counted the children outside and observed 1 staff outside with 20 children and 4 three year olds (3 yr old mixed ratio 15 children to 1 staff). Staff outside said the other teacher was here but did not know where she went. Licensing found the other teacher inside changing a child who had an accident.

Licensing and Child Welfare observed 1 year old classroom teacher on her cell phone in the bathroom with a crying child. Licensing observed the child being upset and crying and not being consoled.
