Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. While LPA was touring the facility, a child was in the restroom, that is located outside of the classroom, in the hallway, while staff #3 was walking back to the classroom. The stall door was also closed and visual supervision was not possible. This poses an immediate health, safety or personal rights risk to persons in care. POC Due Date: 10/12/2023
