Principal conducted a meeting and training regarding incident. Per Principal they will submit a copy of agenda and signing sheet to LPA by POC Due Date Based on information obtained during this investigation, the licensee did not comply in the section above which facility failed to provide a safe environment which caused C#1 who wandered off the facility premises. This poses an immediate health and safety risk to children in care. Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above in that facility did not inform the department of the Principal change within 10 days, which poses/posed a potential health, safety or personal rights risk to persons in care.
