The licensee, states the staff is always watching the children and provided a written plan, of supervision where the staff will make sure children are never left alone. Based on information obtained from the DCFS investigation, and witness interviews Children engagend in inappropriate interactions while in care. This poses an immediate risk to the healt and safety of children in care. This is a repeat violation. A Civil penalty of $250 is assessed.

The licensee has purchased a video and aido monitor and placed in the bedroom where daycare children sleep after hours. Per licensee, they will provide a plan of supervision to ensure children are safe. Based on interview and review of documents. The licensee failed to ensure that children in care are adequately supervised resulting in children injured. This poses an immediate threat to the health and safety of children in care.

102416.2(f)(1) · Any injury suffered by a child in care shall be reported to that child's parent or authorized representative regardless of treatment by a medical professional.

Per licensee, they will ensure to report any child injury to the parents/authorized representaive regarless if the child needs medical attention. A written stamenent was provided. Per licensee interview, Child #1 reported to licensee that they were injured and licensee did not report to the parent. This poses a potential risk to the health and safety of children in care.

Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that there is a fire pit observed in the children's play area in the backyard which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 07/21/2023

Licensee removed the can of Raid out of the areas accessible to children in care. Deficiency corrected during visit. LPAs observed a can of Raid on the coffee table in the living room that is accessible to children in care. This poses an immediate risk to the health and safety of children in care. Safety precautions shall include but not be limited to Per licensee, they will remove items that are potentially and provide proof of corrention to LPA via email by POC due date LPAs observed that the back yard where children play is cluttered, with the grill not covered, tools accessible to children in care, toys that are not clean and not in good condition posing a potential risk to the health and safety of children in care.

102416.5(a)(f) · (a) The capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time.(f) The total licensed capacity for a Large Family Child Care Home shall not exceed fourteen children.

Per licensee, Child #1 was sent home on same date. Licensee provided written declaration confirming the overcapacity and stating they understand not to have more children than the license allows at any one time. Licensee will provide a written children's schedule. Per Licensee's verbal and written admission to LPA that on 7/12/22 she was overcapacity, having 15 children in care. Per licensee, Child #1 was present on a trial basis. This poses an immedicate risk to the health and safety of children in care.
