This req is not met as evidence by Per Prinicipal- they have already conducted meetings and trainings regardincg incident. Staff have been moved around in classrooms and they are meeting with staff relations. Per Principal they will submit a copy of agenda and signing sheet to LPA by 11/8/2024.

Based on interviews the licensee did not comply in the section above in that 4 of 4 staff did not witness incident between child # 1 & child # 2 causing child # 1 hitting their head and lip req. medical attention.This poses an immediate risk for the health and safety of children in care SUPERVISOR'S NAME: Brandi VanOosten

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not me at evidenced by Point- of- use filters were installed on each facuets. Faucets were re-tested on 11/30/2022, passing with no Lead exccedence. The department received results on 3/21/2024 Based on ispection, the licensee did not comply with the directive above, as that Sink faucet F, G, H, J and M tested with an Action Level Exceedance (ALE) of 6.01, 9.390, 8.360, 5.530 and 25.200 ppb.
