DIR will email LPA a training agenda, including staff sign-in on topics of safe sleep, sleep log requirements and staff understanding of Title 22 101429. Based on interviews and record review the facility did not document 15-minute sleep checks on sleeping infants over the last month. This poses a potential risk to the health and safety of children in care.
