    Director states that the facility will suspend the use of the drop-side cribs by June 1st, 2026. The facility shall also remove the cribs from the facility completely by June 5, 2026 and provide photo documentation to the department. Based on observation and interview the facility did not comply with the section cited above as there were drop-side cribs in use and present in the infant sleeping area which poses/posed a potential health, safety or personal rights risk to persons in care.

Director will send a letter to all parents to provide this letter to the department by June 5th, 2026. Based on observation and interview all infants had and used sleep sacks during nap time, and the facility was observed using a sleep sack loose in the crib which poses/posed a potential health, safety or personal rights risk to persons in care.
