Director states that the facility will suspend the use of the drop-side cribs by June 1st, 2026. The facility shall also remove the cribs from the facility completely by June 5, 2026 and provide photo documentation to the department. Based on observation and interview the facility did not comply with the section cited above as there were drop-side cribs in use and present in the infant sleeping area which poses/posed a potential health, safety or personal rights risk to persons in care.

101430(3)(C) · (C) An infant shall not be swaddled while in care.

Director states that they will suspend the use of the sleep sacks going forward and no later than June 1st, 2026. Director will send a letter to all parents to provide this letter to the department by June 5th, 2026. Based on observation and interview all infants had and used sleep sacks during nap time, and the facility was observed using a sleep sack loose in the crib which poses/posed a potential health, safety or personal rights risk to persons in care. The Director, Eva Campbell, will conduct a staff training and will provide documentation of the training agenda and the teacher signatures confirming their attendance, to the Department, by 09/15/2025. Based on interviews, and observation, the facility did not comply with the section cited above in that, the LPA observed 5 infants with one teacher (S2) in the infant classroom, which poses/posed a potential health, safety, or personal rights risk to children in care.

Based on interviews, observation, and documents obtained, the facility did not comply with the section cited above in that, the LPA observed C1 outside on the playground with preschool children, which poses/posed a potential health, safety, or personal rights risk to children in care.

The Director, Eva Campbell, will recreate the menu in accordance to Title 7, Code of Federal Regulations, Part 226.20, and provide at least 2 months worth of menus, to the Department, by 09/15/2025. Based on interviews, observation, and documents obtained, the facility did not comply with the section cited above in that, the LPA observed lunch provided to the day care children that does not meet requirments for meals, which poses/posed a potential health, safety, or personal rights risk to children in care.

101216.4(a) · 101216.4 Preschool Program with Toddler Component (a)...The provisions of Sections 101351 through 101439.1 shall apply for children between the ages of 18 and 24.

The Director, Eva Campbell, will ensure that a fully qualified staff member, with infant units, supervises the toddler children. Based on interviews, observation, and documents obtained, the facility did not comply with the section cited above in that, the LPA observed (S1), who has not completed the infant units, was supervsing (C1), alone, which poses/posed a potential health, safety, or personal rights risk to children in care.
