Deficient Practice Statement Based on observation, the facility did not comply with the section cited above in that, the LPA observed the Preschool Room(Room3) to have 26 children, when maximum licensed capacity is 23 children. This poses/posed a potential health, safety or personal rights risk to children in care. POC Due Date: 10/10/2025

Correction has been made and parents were provided a copy of childs file. Based upon interviews, the facility, denied the parent access to Child #1's file upon disenrollment. This is a potential risk to the health and safety of children in care.

Correction has been made. LPA provided cpoy of regualtion. Director will provide a written statement stating she understands the regulation.
